How to obtain a certified copy of your birth certificate

If you were born in Missouri:

- Bring or mail a completed application, that can be obtained through license bureaus or the health department, to:

In Missouri, birth and death certificates can be obtained at any county health department - it does not have to be the county of birth. Cost is $15.

If you were not born in Missouri:
