Born Aug. 19, 1953, in Stoddard County, son of Pansy Gregory Ezell of Malden and the late Dayton Forkum, he was a self-employed farmer for 30 years and a member of the Bernie Church of Christ.
On Feb. 12, 1971, in Idalia, he married Karen Pyle, who survives of the home in Dexter.
Other survivors include: one son and daughter-in-law, Kenneth and Nicole Forkum Jr. of Bloomfield; one daughter and son-in-law, Doug and Crystal Forkum Moore of Dexter; two brothers, Larry Forkum of Malden and Jimmy Forkum of Forest City, Ark.; and three grandsons and one granddaughter.
Visitation is from 4:30-8 p.m. Monday at the Rainey-Mathis Funeral Home in Dexter, where services will be conducted at 11 a.m. Tuesday. Stan Dauck will officiate.
Burial will follow in the Chalk Bluff Cemetery near Dexter.
