Hazel Forman

Born Oct. 28, 1935, in Amboy, Ill., daughter of the late Lyle K. and Florine E. Wheaton Etheridge, she owned and operated Century Nursing House before retiring. She was a member of New Life Christian Center in Morley.

On March 28, 1953, she married Joseph Gilbert Forman who preceded her in death on June 15, 2004.

Survivors include: one son, Randy Joe Forman of Lincoln, Neb.; four daughters, Pamela Massey of Sikeston, Jody Burgess of California, Jeana Wathen of Fredericktown and Jana Ross of Matthews; three brothers, Dale M. Etheridge of Rock Falls, Ill., Ted Etheridge of Amboy and Kermit Etheridge of Mendota, Ill.; one sister, Joan Etheridge of Huntington, W.Va.; and 10 grandchildren and five great-grandchildren.

Her twin sister, Helen Reed, preceded her in death.

Visitation is scheduled from 5-8 p.m. Monday at Ponder Funeral Home in Sikeston where services will be held at 2 p.m. Tuesday.

Burial will follow in the Matthews Cemetery.
