CHAFFEE -- Jamie Elaine Koch, 48, died Oct. 18, 2014, at Mercy Hospital in Springfield.

Visitation is from 4-8 p.m. Tuesday at Amick-Burnett Funeral Chapel in Chaffee, with parish prayers at 4 p.m. A funeral Mass will be at 10 a.m. Wednesday at St. Ambrose Catholic Church in Chaffee with the Rev. Randy Tochtrop serving as celebrant.

Burial will follow in the St. Ambrose Catholic Cemetery.
